Joint Commission - Pioneer In Quality Award

Pioneers in Quality (PIQ) is a Joint Commission program to assist hospitals on their journey towards electronic clinical quality measure adoption that includes educational programs (e.g., webinars for CEUs), a resource portal, recognition categories, an advisory council, a modified annual report, speaker’s bureau outreach, a peer – to – peer solution exchange, as well as having a strong focus on partnering with hospitals to provide the highest level of quality care for patients and their families.


Healthgrades - 2016 Patient Safety Excellence Award

If you need to be admitted into a hospital, the last thing you would expect is to become unnecessarily sicker. Patient safety measures how well a hospital prevents injuries, infections, and other serious conditions based on 14 serious, potentially preventable adverse events. The Patient Safety Excellence Award recognizes hospitals that have the lowest occurrences of 14 preventable patient safety events. These hospitals are in the top 10% in the nation for patient safety.

2017 Excellence in Action Award Winners - Grace Ridge Retirement Community and Grace Heights Health and Rehabilitation Center

Grace Ridge Retirement Community and Grace Heights Health and Rehabilitation Center have earned the NRC Health Excellence in Action Award, acknowledging resident/family responses on 2016 surveys and the positive answers for "Would Recommend Facility." To qualify for the award, facilities had to have a 30 percent response rate on surveys; scored in the top 10 percent in assisted living and independent living facilities; and scored 20 percent in the skilled nursing performance as compared to other qualified facilities in the NRC Health database. We are proud to recognize our dedicated staff for their compassionate care.

Carolinas HealthCare System - Touchstone Awards: Gold & Silver

Touchstone awards, the highest honor one can receive for improving the patient experience, patient safety, clinical outcomes and efficiency at Carolinas HealthCare System. Only 84 out of 240 applicants for award projects received a Gold, Silver or Bronze award in 2016.

District 1 nurse aide of the year
Libby Tallent, CNA, at Grace Heights Health and Rehabilitation Center, was named District 1 Nurse Aide of the Year from the North Carolina Health Care Facilities Association. The recognition is an important part of the long term care community’s effort to recognize the valuable contributions of front-line staff to the quality of care and quality of life of individuals living in skilled nursing facilities. Winners were nominated by their colleagues for this special award.
