Hospitalization can be a stressful time for patients and their families, and planning for care after discharge can also create anxiety. That’s why Carolinas HealthCare System - Blue Ridge’s case management team, composed of registered nurse case managers and experienced social workers, work closely with physicians using a multidisciplinary approach to make our patients’ stay and transition out of the hospital as pleasant, smooth and successful as possible.

Hospital Case Management and Social Services staff are available to help you and your family deal with physical, emotional, social and financial issues associated with your hospitalization and post-hospitalization. These services are offered at no charge. A nurse case manager is available to coordinate care, ensuring that the total needs of the patient are met. The social worker coordinates discharge planning after care needs.

 

 

At Carolinas HealthCare System - Blue Ridge, we believe planning your recovery is essential. Upon entering the hospital, we begin planning for your discharge. This includes disease management, referrals to community agencies, assisting with transfers to rehabilitation facilities, rest homes, or nursing homes, also referrals for home health services and home medical equipment. The Case Management Department will work with your physician to ensure that proper arrangements for ongoing care are made before you leave the hospital. As time draws near for you to leave the hospital, assistance may be needed with the following:
