medsHelping You Manage Troublesome Medications

Carolinas HealthCare System Blue Ridge recently expanded its clinical pharmacy services by opening a Medication Management Clinic to provide monitoring and regulation of medicines used to prevent blood from clotting (anticoagulation therapy).

The Medication Management Clinic is staffed by experienced clinical pharmacists who are certified in anticoagulation management and can assist providers and patients in the use of anticoagulants such as:

  • Coumadin® (warfarin)
  • Xarelto® (rivaroxaban)
  • Eliquis® (apixaban)
  • Pradaxa® (dabigatran)

The purpose of the clinic is to provide patients with information regarding anticoagulation therapy, potential drug interactions, dietary vitamin K review and signs/symptoms of bleeding. We also provide enoxaparin therapy, education and monitoring.

Hours: Monday, Wednesday and Thursday from 9 a.m. to Noon

Physicians, physician assistants or nurse practitioners may refer patients to the clinic by calling 828-580-7955.
